Virtual Visits with MDLive allow you to talk with a doctor by phone or video 24/7 and are free for Blue Choice PPO HDHP, Blue Essentials HMO, and Blue Premier HMO plan members. You can meet with the next available professional or make an appointment.

The program offers confidential, professional support and resources at no cost to you and your family (any member of your household and your children up to age 26, even if the child does not live with you).

Benefits Enrollment

You are responsible for completing online enrollment, even if you decline all coverage, within 31 calendar days of your date of hire. Failure to enroll within this time frame will result in forfeiture of benefits until the next open enrollment period.
Changes are not permitted unless you experience a qualifying event and request a change of election within 31 calendar days of the event. Please review the for additional information.
Full-Time Employees: Premiums are due beginning on the first day of coverage. Therefore, a delay in enrollment may result in an increased deduction on the first paycheck following successful enrollment in order to collect any premium(s) not previously deducted. The first payroll deduction of premium(s) is determined by the 麻豆女郎 Pay Schedulepdf (2023-2024)pdf (260/261/262-day employeespdf(2023-2024pdf)). You may refer to the Benefits Enrollment/Deduction Schedulepdf (2023-2024pdf) for more details on these timelines.
Part-Time, Substitute, and Temporary Employees: Premiums are due by the 20th day of each preceding month (i.e., November premium due by October 20). If you are enrolling on or after the 20th day of the month, you must submit payment for one calendar month immediately upon completion of your enrollment. All premiums must be mailed to the District鈥檚 third party administrator. If you fail to pay the monthly premiums in a timely manner, the District will proceed with the coverage cancellation process. Effective September 1, 2019, you must work 52 hours per month to maintain eligibility for the medical plan. To meet this requirement, substitutes must work a minimum of six full workdays each month.

Take the following steps to log in to FFenroll and enroll in or decline the benefits offered to you:

3. Enter your six-digit PIN: last four digits of your SSN, followed by the last two digits of your birth year (Example: If the last four digits of your SSN are 1234 and you were born in 1970, then your PIN will be 123470).
4. Click the ‘Log in’ button.

You do not have to be on the District network to enroll.

Form 1095-C

For employees who do not consent to electronic delivery of Form 1095-C, a paper copy of the 2023 form was mailed February 29, 2024, to the address of record. It is important to keep your mailing address updated with the District. If you have moved and are a current employee, please update your address in the Employee Access Center. If you have moved and are no longer an employee of 麻豆女郎, complete an and submit it to the 麻豆女郎 Human Resources Department.

Please note the IRS due date for distributing 2023 forms is March 1, 2024.

Additional Form 1095-C copies are available online through the Employee Access Center. The first tax year requiring employer distribution of 1095-C forms was 2015, so forms for prior years are not available. If a replacement Form 1095-C is needed, please contact the 麻豆女郎 Employee Benefits Office at benefitsoffice@conroeisd.net or 936-709-7808.

What is a Form 1095-C?

Form 1095-C is a tax document prepared by the District that reports its offer of employer-provided health coverage to you and your dependents, as applicable.
The form is also used to report the individuals enrolled in the District’s group medical plan.
You may be required to submit information from Form 1095-C on your federal tax return.

Who receives a Form 1095-C?

Employees who enrolled in the District’s group medical plan for one or more months in a calendar year receive a Form 1095-C. Only one form is provided and includes all the individuals listed on your policy; you may need to make copies for your spouse or dependents, as necessary.
Employees who worked 130 hours or more at least one calendar month in a calendar year receive a Form 1095-C, even if they were never enrolled in the District’s group medical plan.

What if a Form 1095-C is not available for me on EAC?

This means you either never enrolled in the District’s medical plan, or you never worked 130 hours or more in a calendar month in the calendar year.

Does this form mean I am being audited by the Internal Revenue Service (IRS)?

No, this form does not suggest an IRS investigation.
The IRS requires employers to provide this form to all applicable employees for tax purposes.

What do I do if something is wrong on my Form 1095-C?

Your Form 1095-C can be corrected and reissued.
